Who is covered by my membership?
Your ChiroCare membership covers you, your spouse and any
children under 23 years old living in your household.
Why isn’t a Chiropractor I want to use currently in the Network?
ChiroCare has selectively contracted for participating Chiropractic
networks. If you wish to refer a doctor to the participating network,
call our Member Services Representative at 1-866-209-0070 and we
will pass your referral on to the participating network.

The ChiroCare card enables your entire immediate family, including dependent children to age 23, to access participating Chiropractors.

General Instructions
The membership card you received with this member guide is
for you and your family to access Chiropractic services at
participating Chiropractor offices. You have been issued two
cards. Should you lose your cards, call our Member Service
Representative to order a replacement card. Replacement
cards will be billed to you at $10.00 per card. |
To locate a participating doctor, call our Member Service Representative at 1-866-209-0070 or go to the CNS website at www.chirocareplan.com and search under the directory link.
The membership card instructs the doctor’s office about the correct discounts and financial arrangements for you as a ChiroCare patient. Always present the ChiroCare card to the office staff when having covered services.
Payment of the discounted amount is made to the doctor at the time of treatment. You may see the doctor as often as you need to.
ChiroCare is not insurance, so there are no forms, no maximums, no exclusions for pre-existing conditions, and no deductibles.
