How to Participate
Licensed Doctors of Chiropractic, who maintain acceptable levels of malpractice coverage and are not encumbered by limitations to their practices, may participate in ChiroCare.

Your questions will be answered and we will explain how ChiroCare operates.
Once you are satisfied with participating in ChiroCare, you can either complete the application contained on our website or we will send one to you when you contact us.
Send a completed application along with a copy of your license and the declaration page from your malpractice insurance agreement along with a check for $50 to:

The $50 is to cover the cost of verifying the information from your application and your credentials. This is a one time fee; you will never be billed for any additional credential updating.
You will also find a standard ChiroCare Participating Agreement on the website. Print that and sign it and send it along with the application to the above address. Your signed, executed participating agreement will be sent to you via surface mail within 30 days of receipt at our offices.
Along with the executed agreement, you will receive a supply of ChiroCare applications and sample brochures for you to place at your front desk. We ask that
your office staff be sure that the applications from your office include your practice name in the appropriate place on the application.
All renewals will be initiated by us. We provide you with a copy of our renewal letters to
your patients annually.
